On-line Patient Access

We ask our new and existing patients to please create an account through our patient portal. It’s easy to create and allows you to take advantage of online services offered to our patients. After you create your account, you will receive an e-mail from us that includes your user ID and a link back to our patient portal. You may log in at any time to begin using our services. Once you have an established account, you gain access to forms to complete before your visit which will save you time and access to lab reports. In addition, you can inquire about appointments, request prescription refills and ask questions to our staff.

We do ask our patients to complete a health history form prior to each visit. Registration forms need to be updated only if a change in contact information and or billing/insurance information has occurred since your last office visit.

We participate with many different insurance plans and will file for reimbursement. It is your responsibility to know the details of your health insurance plan with regard to referrals, co-payments and percentage of coverage. We require your co-payment at the time of your visit. In addition to cash and checks, we also accept VISA, MasterCard, and American Express.

We accept the following insurance providers:

Aetna US Healthcare
Blue Advantage
Blue Cross Blue Shield
Beechstreet
Champus/Champus Tricare
Cigna
Coventry
First Health
First Medical Network
Great West Healthcare
Humana
Medicare
Multi Plan
Nova Net
PHCS PPO/Open Access
United Healthcare

All laboratory procedures are billed separately from your office visit. You could receive a laboratory bill from the following:

LabCorp: 1-800-845-6167
Genzyme (Genetic testing only): 1-800-326-7002

If you receive a bill from the lab listed above, please call the listed number for questions. If you still have a question and need additional help, please do not hesitate to call our office.

Deductibles are paid out of pocket each year before insurance kicks in.

Each family member usually has a separate deductible.

Co-payments are flat fees charged each time you visit the doctor or use any medical services, regardless of the cost of the procedure.

Co-insurance requires you to pay a percentage of the total cost of the care.

Many plans require either co-insurance or co-pay but some charge both. Maximum out-of-pocket expense is the most you’ll have to spend before all of your medical bills are covered.

On-line forms are being updated at this time. You may complete your patient forms at our Dothan or Enterprise office.
