Welcome New Patients

We are so happy you have chosen Ladies First as your OB/GYN provider! The trusted physicians and staff at Ladies First strive to show compassion and care to our patients. We are continually making efforts to improve the patient experience.

Our Dothan office is open from 8:00 am to 5:00 pm, Monday – Friday. Each doctor’s hours vary from day to day. We ask that you call in advance for an appointment. We make a conscious effort to see patients on time. However, the occasion may arise when emergencies or other unforeseen problems result in delays. We appreciate your understanding when this happens.

Before your appointment

We encourage our new patients to arrive 15 minutes earlier than their scheduled appointment time. This allows for time to complete the new patient forms. You are welcome to download, save, complete and print the forms below ahead of time and bring them with you. Also, please have medical records and/or notes from other physicians ready for the receptionist.

What to bring to your first visit:

  • A valid form of I.D.
  • Current Insurance Card
  • Copay
  • All of the above Registration Forms
  • Pharmacy information of your choice

Running late for your appointment?

If you find you are running late for an appointment, please call us to let us know.  If you are over 20 minutes late, we will make every attempt to accommodate you into our schedule; however, you may need to wait. You also have the option to reschedule your appointment.

A federal law known as HIPAA protects patient privacy and prohibits us from disclosing or discussing personal information or treatment regarding a patient without the patient’s written authorization. This applies to children/adults/spouses/partners – no exceptions.  Verbal authorizations are not acceptable.

Medical Records are the property of the medical practice. Requests for a copy of a patient medical record should be made in writing. This request must be HIPAA compliant; and must identify the patient including date of birth, current address, and signature. The request must also include where the records are to be sent with full name and complete address. Please allow 5 business days for medical records processing. A $25 fee applies to the copying, mailing, and storing medical records.
